
| What Does It Cost? |
To Join the Ambulance Service Welfare Fund, subscriptions are taken by the Ambulance Service as payroll deductions. The amount is $13.00 per fortnight for operational staff and $9.00 per fortnight for non operational staff. From this amount, 5/6ths fo that is returned to you by claiming on your tax each financial year. Therefore is costs approximately $56/$39 per year for all of the benefits available to you. |
Currently this membership is only available to full time employees of the SA Ambulance Service. There is not another similar type of fund in Australia that can offer all these benefits for the low cost of $56/$39 per year. |

| We have Casual, Volunteer and Past memberships available to you at an annual cost of $44.00, which entitles you to use of the holiday homes and the houseboat, corporate rates on Health Insurance and the DEFCOM card discounts. In addition, you are entitled to the hire of baby capsules / breast pumps/nebulisers, Entertainment Books, discount movie tickets and other member special offers. |
| Exclusions for casual, volunteer & past members are financial assistance, sickness claims & death benefit. |
| How Do I Join? |
It's simple, just click here for the Registration Form and return it to the Ambulance Service Welfare Fund (fax or email) or contact us on 8340 3511. We will send you a membership card, together with discount cards and vouchers and information about what we have to offer you. EASY! |

| How Do I Claim? |
To claim sickness and accident benefits, you need to fill out a claim form (download here) and forward it, along with your Doctor's certificate to the Fund Office. Payment will be forwarded once the claim is approved. All forms can be found online at the forms page. |

| How Do I Join The Health Fund Plans? |
To join any of our Health Plans just give us a call and we will send you out details of both Health Providers. If you are already a member of one of our providers, then contact our office and we will advise you of the contact details of our Account Managers to receive the additional benefits available through Welfare Fund membership. |

| How Do I Book The Holiday Homes? |
Go to the holiday homes page and click on subsequent links to view updated availabilities, booking forms, property descriptions and photos. You can submit a booking request using the online booking request forms on each home's corresponding page, or phone us at the office (8340 1800) to make a booking. Bookings will be considered 'pending' until receipt of a completed form. The holiday homes are popular and peak periods need to be booked well in advance. |

| What If I want a Baby Capsule / Nebuliser ? |
Just contact the office and we arrange one for you when you need it. It is better for the office if you give a few weeks notice for the Baby Capsules, otherwise you will have the office staff in a fluster hijacking one for you! |
